“Why do some under-eye treatments need 2ml of filler, while others only need 1ml?”

The answer comes down to facial structure, volume loss, and how the under eye connects to the cheek — not just how “deep” the tear trough looks.

Understanding the Tear Trough

The tear trough (under eye hollow) isn’t just a line — it’s part of a wider structure involving:

  • The under eye ligament

  • The mid-face (cheek)

  • The transition between these two areas

When this area loses support, it creates:

The Key Difference: Structure vs Surface

When 1ml Can Be Enough (0.5ml per side)

👉 Total: 2ml (1ml per side)

Why 1ml Isn’t Always Enough

If we only used 0.5ml per side in these deeper cases:

  • We would only soften the transition, not correct it

  • There wouldn’t be enough volume to support the cheek structure

  • The result can look like the under eye is being “extended” downward, rather than blended naturally

In simple terms:

You’re treating the symptom (the hollow), not the cause (lack of support).

The Importance of Cheek Support

A key principle in natural-looking results:

👉 The cheek should sit slightly in front of the under eye

  • The under eye looks smoother

  • The face looks fresher — not overfilled

In many cases, this means:

  • Starting with the cheek

  • Then blending into the tear trough
